Instructions for
Volunteer Hours Form
Please fill in the
volunteer hours form as follows:
Student Name: List the name of the student
turning in the form.
Parent Name: List the name of the parent
responsible for that student.
Family e-mail: It is important to list an
e-mail address as any discrepancies or errors will be notified by e-mail or by
telephone.
Student Names: List ALL siblings and
their homerooms otherwise hours may not be credited to all family members.
Volunteer Task/Hours/Given/Date/Approval: Please
list the task or donation for which you are requesting hours. Examples would be
field trips, PTSO events, or donations to a teacher. If you are making a
monetary donation, please list the amount given. NO HOURS WILL BE GIVEN WITHOUT
AN AUTHORIZED SIGNATURE IN THE APPROVAL SECTION. All donations need to be
brought to either office and will be signed for there. If the donation is for a
specific teacher it will be given to them after it has been signed off on by the
office.
Total Hours Required: Please list the number of
hours required of your family. This
means 30 hours for the first school and 10 hours for each
additional school.
Hours Remaining: Please list the number of
hours that you believe remain to be completed. If there is a discrepancy with
our records you will be notified by e-mail or by telephone.
Receipt Attached: Please attach all receipts
where possible.
Donated to: List who you would like
to have your donated items go to.

Do not forget to keep the yellow copy for your records.
One form needs to be filled in per family per month. Hours should be reported
in the month they were completed. Additional forms are available in both
offices and at PTSO events. |
